Program Guidelines

All treatment must be paid in full at each visit to keep the plan in effect.
There will be a $50 reinstatement fee if your plan lapses for 9+ months.
Non-Transferable, Non-Refundable.
Patients with traditional dental insurance are not eligible.
Fees are due on or before the first appointment of the year.

Program Exclusions and Limites

Cannot be used in conjunction with another dental plan, dental insurance, or financing.
Treatment is at the sole discretion of the Little Grins Pediatric Dentistry.
This plan is only honored at Little Grins and can not be used at any other office.
Plan does not cover treatment due to an injury involving a lawsuit, car accident, or dental care covered by medical insurance, workman’s compensation, or homeowner’s insurance.
Family members can enroll at different time of the year, but the end of the benefit year will remain the same for all family members; 365 days from the f irst family member’s enrollment date.
